My Health logoMy Health is the name of the voluntary Wellness Rewards Program that the City offers in conjunction with the medical and prescription drug plans.  It is your choice whether you want to participate in this program or not.  

With participation in My Health, you'll receive a variety of rewards for committing to a health-conscious lifestyle.  In exchange for those rewards, you (and your covered spouse or qualified domestic partner) must maintain all of the requirements below.

 1) Have a health screening at The Center every 12 months.

 2) Be physically active by completing 60 minutes of exercise every week and submitting a Physical Activity Affidavit every month.

 3) Health Education credit once per quarter, either by attending a class or by completing a Quarterly Education Reporting Form after viewing one of the alternate approved educational materials.  Please send completed forms to Employee Benefits by either faxing 215-2298 or emailing to [email protected].

 4) Actively manage your chronic conditions by meeting with your Health Coach.

 5) If covering children, provide proof of annual well-child care visit.

If for some reason you are removed from the program and wish to re-join, you (and your covered spouse or qualified domestic partner) must complete a health screening, attend quarterly education, and submit two consecutive months of Physical Activity Affidavits before you can re-enter the program.  Your My Health status will be renewed on the 1st of the month following completion of the form.
